Online 'Pre-Registration' With The Practice

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us.

*** Please note:  You will need to come to the surgery in person with two forms of identification - one photographic ie passport/driving licence and one proving you live at the address you are registering with ie utility bill/tenancy agreement - BEFORE your registration will be accepted ***

If you live within our Practice Area then you will be able to register with the practice. If you live outside the Practice Area you may still be able to register with us as part of our Out of Area patient scheme. If you live outside of the boundary please ask at reception to see whether you are able to register in this way. Children under the age of 16 are not able to be registered on the Out of Area registration scheme. 

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct and to ascertain what is your preferred method of communication. If you have any specific communication needs that we need to be aware of please inform reception at this timeYou will be required to present two forms of identification, one of which must be photographic.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
